Steps for Freeze/Cancellation
1. Complete the below form
2. Once received, one of our FAC team members will reach out to you in 3-5 business days for confirmation of your request
3. Upon confirmation of freeze/cancel the appropriate form will be sent for you to sign and return
4. Once the Freeze/Cancel form is received, we will complete your request and update your account
*Annual Agreements are subject to an early termination fee of $70 or 10% of the total contract price which ever is greater not to exceed $150.
*All freezes are subject to a $20 administrative fee.
